Community Nurse

Main area                    Mental Health Nursing: Community

Grade                          NHS AfC: Band 5

Contract                      Permanent

Hours                          Full time - 37.5 hours per week (Monday-Friday 9-5)

Location                      Various sites across Sussex, including Hove and Chichester

Salary                          £25,655 - £31,534 per annum

Main duties of the job

Our community teams work with adults living with mental illness, supporting their recovery and the development of skills and strategies to support them to maintain their wellbeing. You will have a good knowledge of working with people with severe and enduring mental health problems and be able to demonstrate clinically effective care management skills and interventions in accordance with the Care Programme Approach.

You will be working as part of an MDT including social workers, psychologists and medics the post holder will provide specialist nursing interventions and act in the role of Lead Practitioner for a defined caseload of clients. The work is varied and interesting and all team members are involved with the full range of best practice interventions.

 

Detailed job description and main responsibilities

To participate within the multi-disciplinary professional team in the relevant care group to deliver safe patient care focussing on therapeutic relationships.

To work in partnership with families and carers to ensure the patient’s individual health needs are met.

To have an understanding of nursing interventions relevant to the care group to include education, clinical care, wellness recovery action plans and discharge planning.

Qualifications

Essential criteria

  • Registered Nurse with current registration [mental health/LD/Adult], appropriate to the job role.

Knowledge/Experience

Essential criteria

  • Knowledge of MHA (1983/2007).
  • Knowledge of Mental Capacity Act (2005), to include practical knowledge of FACE documentation, DOLS and Best Interest decisions.
  • Child and Adult safe guarding procedures
  • Change management at organising and prioritising their workload
  • Demonstrate ability to enhance care through evidence based knowledge
  • Demonstrate professional development relevant to the specific care group
